چکیده

Abstract PURPOSE Differentiation of tumor progression (TP) from pseudoprogression (PsP) is a major unmet need in post-treatment glioblastoma (GBM). 18F-Fluciclovine synthetic amino acid PET radiotracer with higher uptake tissue vs. areas treatment-related change. We investigated the value for differentiating PsP TP independent and combination multi-parametric MRI. METHODS prospectively enrolled 30 patients GBM new or enlarging contrast-enhancing lesion on MRI after chemoradiotherapy who were planned surgical resection lesion. Patients underwent pre-operative Following surgery, relative percentages viable therapy-related changes observed histopathology quantified. categorized as if represented ≥ 50% specimen, mixed < > 10%, ≤ 10%. RESULTS 18 had TP, 4 8 PsP. TP/mixed significantly 40-50 minutes SUVmax (6.64 + 1.88 vs 4.11± 1.52, p=0.009) an cut-off 4.66 provided 90% sensitivity 83% specificity differentiation (AUC=0.856). A maximum cerebral blood volume (CBVmax) 3.67 71% (AUC=0.779). Combining 4.662 CBVmax 100% 80% (AUC=0.95). The time activity curve patterns to peaks not different between groups. Normalization parameters normal brain parenchyma helpful differentiate groups due variability subjects. CONCLUSION can accurately patients, even more accurate achieved when combined

Late Pseudoprogression in Glioblastoma: Diagnostic Value of Dynamic O-(2-[18F]fluoroethyl)-L-Tyrosine PET.

PURPOSE Pseudoprogression (PsP) is characterized by therapy-associated but not tumor growth-associated increases of contrast-enhancing glioblastoma lesions on MRI. Although typically occurring during the first 3 months after radiochemotherapy, PsP may occur later in the course of the disease and may then be particularly difficult to distinguish from true tumor progression. We explored PET using...

Differentiation of pseudoprogression and real progression in glioblastoma using ADC parametric response maps

PURPOSE The purpose of this study was to investigate whether a voxel-wise analysis of apparent diffusion coefficient (ADC) values may differentiate between progressive disease (PD) and pseudoprogression (PsP) in patients with high-grade glioma using the parametric response map, a newly introduced postprocessing tool. METHODS Twenty-eight patients with proven PD and seven patients with PsP wer...

Accuracy of tumor segmentation from multi-parametric prostate MRI and 18F-choline PET/CT for focal prostate cancer therapy applications

BACKGROUND The study aims to assess the accuracy of multi-parametric prostate MRI (mpMRI) and 18F-choline PET/CT in tumor segmentation for clinically significant prostate cancer. 18F-choline PET/CT and 3 T mpMRI were performed in 10 prospective subjects prior to prostatectomy. All subjects had a single biopsy-confirmed focus of Gleason ≥ 3+4 cancer. Two radiologists (readers 1 and 2) determined...

Impact of attenuation correction on clinical [18F]FDG brain PET in combined PET/MRI

BACKGROUND In PET/MRI, linear photon attenuation coefficients for attenuation correction (AC) cannot be directly derived, and cortical bone is, so far, usually not considered. This results in an underestimation of the average PET signal in PET/MRI. Recently introduced MR-AC methods predicting bone information from anatomic MRI or proton density-weighted zero-time imaging may solve this problem ...
